Bankruptcy

I have a Chapter 7 bankruptcy  discharged on all my credit including two mortgage loans.   I ordered a copy of my credit report ,  both mortgages  were included in bankruptcy. But, noticed one lender changed the account number on the mortgage loan and has it listed on my credit as an open account with "Pays account as agreed."  

When it should read "Included in Bankruptcy"  What are my options?    Is this loan discharged or am I responsible for the loan?  Is this something I can dispute?   Will the dispute affect my FICO score?
