I went through the app and I got to the point where it asked me to add additional users and eventually it said we need more information, we'll contact you or something along those lines. I found out you can check the app status online and when I checked it it said it was declined. Is this common? The only other thing is that I think my Costco membership is expired as of February 2013. Any ideas?
