For those that have this product: (1) how long from initial approval did it take to get account setup; and (2) how long to start reporting?

It doesn't take long. You'll get the paperwork in the mail, unless you opted for picking up at a local  branch.

 

As for reporting, it was before 30-days when they started to report mine.
