American Express 3X CLI Question

I recently ticked over 61 days with my new Amex Everyday card.  I successfully upgraded my limit from $4,000 to $12,000.  Very happy with that.  I also have a Costco Amex with a low $1,500 limit that I use fully and PIF just about every month (three kids eating like mad and the whole family's gas...I shop at Costco a lot).  I am wondering if one can only request the 3X cli on one card per limit increase period (i.e. pick your highest limit Amex revolver so you get the biggest bang for buck), or if I can also request an increase to $4,500 on the Costco card as well.

Do the lines have to be open a certain amount of time to move the credit around? Thought I saw somewhere 13 months.

The 'from' card has to be open 13 billing statements (and remain open during the reallocation) ; the 'to' card must be open at least 60 days.
