BOA Cash Rewards Denied - Secured Card Offered

So I app'd for the BOA Cash Rewards last week and was denied and offered the secured version after I was denied. I foolishly closed out and regret it cause I want to take advantage of that. 

So today I get a letter in the mail for the offer. The only way to take advantage of this offer is by mail? There is no phone number, no onine offer code, nothing. Just mail your personal info off via the USPS like its 1992? Is this the only way?

you could always call BoA and see if a rep could help you, or go by a branch if there is one near you. Not sure if either could get you help, but I'd at least start there

Oh, I tried calling. One clueless rep after another with no resolution. They must not pride themselves on customer service.
