PS - I am pretty sure that it was the 4506-T form they wanted. They had demanded 3 years of tax returns and wanted unlimited access to my tax returns with the IRS.
And for the guy who had my ss number -- all bogus. Near as I can figure, from talking to many different people at the cc, was someone else uses the same address I do. No one else shows up on my credit report. No identity theft. And yeah, other people live here too! I still get mail for several owners ago!
