AFAIK, they would only charge a fee if they also counted it as a cash advance (in which case the fee is only part of the problem!) Some cards might, as the MCC might come back as financial instituion, and some cards, for example Citi Visas, will often treat such purchases as cash advance

I should have prefaced that by saying: IF the story is true (that the ccc charges a fee) that would be why, but no idea if that happens. Much more likely that the loan holder charges a fee to offset the swipe fee they have to pay.
