If you are a Prime member and go into your Amazon wallet the card details for your Amazon card will show your earning rate and indicate that you eligible to earn 5%. After clicking on "Learn How" and entering your credit card number, it upgrades that card in your Amazon account and shows the earning rate at 5%. From what I have read online, it appears that Chase will automatically mail out new cards.

It looks as though we who have the "Amazon rewards Visa Signature Card" will not be upgraded to the "Amazon Prime Rewards Visa Sig" and that we will only be upgraded to getting 5% back. This means no cool metal card and no card with no FTF.

Also, after upgrading in my Amazon wallet, the picture of my credit card changes from the regular Amazon card to the Amazon Prime card and reflects a new expiration date as well.

I posted this link a few pages back, but the fine print on Amazon's website says if "You link a different card account to your Prime membership." I.e., the card you use to pay your prime membership.
