Neat looks like a standard cc app.

 

I wonder why they have to email it?

You print, fill out, then fax it back right? I would imagine its for your signature to be laser etched that they process it like that.

If you dont mind me asking what is yoru Annual Income, if you do not feel comfortable putting that on here thats fine.

I don't think income is a significant factor for this card. I could be wrong of course.
