I had a US Bank account, but I closed it and my reserve line balance turned into a loan. The high balance during October was $2,600, but
Its paid down to just $104.
I have a serious delenquency with Capital One from 2009 (about 120 days), but I paid it off as agreed and closed the account in early 2010. Then I got A First Premiere Bank Card and used it for a year, then I closed it when I got another Capital One Card.
I also think you should wait until you get your util down. If you apply now and are approved, they will probably give you a low limit. Better to wait until util is down and get a higher limit, especially since you have four other prime cards with $5000 limits.

Should I app now or wait? I have been denied in the past and I know they will pull EQ. My correct scores are 658 (CreditKarma) and 645 (EXP) 661 (TU) 678 (EQ) via USA credit monitoring. I have 4 prime cards with $5k limits and few store cards with limits above $1500. However, I'm concerned with the number of inquiries and baddies. The youngest is from 2009 and oldest is 2007. 2 of the baddies will fall of in July 2014. I have 11 inq's with the youngest being October 2012 and the oldest being Sept. 2011. I have not missed a payments or been late in over 3 years.
What are my chances of approval??
