That's a system issue though - just as likely to happen through the warehouse (it just wasn't your case). I applied for mine online and everything went through just fine.
Maybe. I applied online...not an instant approval. Called in the next day and AMEX wanted to confirm ID. Once I did that, I was approved for $3500. I will be going for the 3X CLI on 11/1 (181 days since the BCE's 3X CLI approval)

Had no problems with mine but you can't establish a trend based on a single data point either way.
I dunno. I double app'd for two cards, not the TE. After making me wait five days they declined the second account and did a HP. Pretty frustrating. They even told me when I called in before the five day period was up that everything looked good. Don't know what's up with that. Must be waiting to see how I handle the one account first.
Soooo... who knows. Costco TE might not happen for me in January.

I would disagree (of course, as a BCE hater) but then I see in your sig you have two of the better alternatives anyway (such as Sallie Mae, AARP). Get the Amex cards for backdating, or for Amex protections, but not for outstanding rewards!
