Chase sends a prepaid envelope to send back to them for your card to be destroyed because they sent me one when my AU's name was spelled wrong. Just call them or go in to a branch.

It must be taken into a Chase Branch, and cast back into the fiery chasm from whence it came. Or use tin snips.
