When I was put on DW's Costco Amex as AU, they also asked for my address, I believe. no SS #, tho.

It varies by lender. Some connect the dots, others don't.
Different address and especially with different name might well not report, unless there's a shared address listed in the past addresses.
Which card/ bank/ lender are you considering? --or who only asked for the name?
You might want to edit your thread title on the original post, something like
JC Penney: Adding an authorized user with just a name?
All you have to do is go back to your original post, click Options over on the right, select Edit, and place your cursor at the beginning of the Subject box. Then you could add the question mark at the end, if you want to get fancy.
I don't know if JCP reports AU's, but that would be a way of catching the eyes of those who do know.
My DW added me as an AU on her JCP card with just giving my name and it started to report on my CR with the next billing cycle!!
