We've had reports of marginal/borderline apps being made, and a HP being done anyway almost always on a non-EX bureau IIRC.  The result was a decline.

It's rare, but an application can result in a HP without it's being an approval, it's just not very likely statistically and if it does, you know you were really close regardless.
