Well, I am not sure that you should write a dispute letter or just use a simple online form through myFICO website. I found it useful to me because I am not good with writing (English is not my primary language). And I don't think that Credit Report Agencies care how good your writing skills are. Here what I did.
Make sure your report is still access able.
Login your myFICO account. Click on the report you wanted to view.
Example: First report should be TransUnion.
You are going to see FICO Score Summary tab (1),
or Understanding Your Score tab (2)
You need to click on Next Steps tab (9)
You'll see "Print Your Report"
Under that you'll see "Find and dispute errors on your report" click on that
Then you have to select what report you wanted to work on.
Follow step by step and instructions. Select items you want to dispute. Choose pre-selected reasons or type in your own explanations.
It will generate a dispute letter. You will have a chance to review it and print it out with address where you should send your dispute letter to. It's so simple and helpful. Hope you can find what you wanted. Good luck.
