Does anyone have a good letter template for disputing addresses on CR?

I am needing to send a letter to the CRA's with corrections to my addresses on file.  Also, oen of them has an incorrect SSN listed for me.  What should I send in with the letter as verification?

I wouldn't even bother with a dispute.. just call and they will generally update addresses right over the phone.

I can't seem to find the report number on these CR's I pulled from myFICO.  Where is it found?

I tried to dispute addresses with Experian a while back. One of my previous address, they have it 6 times. The only differences in the address were that some reported "AV" instead of "AVE" or "FLOOR 2" instead of "APT #2"

They said that they can't change that because they report that information from my creditors.

TU and EQ are pretty easy with disputing address process.
