I agree, i just sent out 9 of them by mail yesterday. It took me about an hour to get all the info and a standard letter written.
Made copies of each one and signed the originals. Also saved each letter in word format.

What I want to know is who is responsible for proving the DOFD?  I know these two accounts are older than 7.5 years, but I have no way to prove it...
