Litigation for a medical collection?

I just received a letter in the mail yesterday from a CA regarding a medical collection account that dates back to 2003.  This is the first communication I have received on this in years.  It states if I don't pay this in 15 days they will reccommend immediate litigation.  It is unpaid.
 
Should I expect this to happen?  Will I be sued?  The amount is $800. I am in a mortgage process now and obviously this concerns me greatly.
 
Any suggestions - I appreciate your help?!!

I swear they 'monitor' to see what you are doing in life and then use whatever they find to twist your arm. 
 
I am thinking SOL as a defense - please check for your state.

Ok it is 5 years old.  Look up the SOL in your state for such debts.  In my state it is four years.  If it has passed the SOL, they can't sue.

I am not a legal expert- BUT
I take this as a threat of legal action.  Under FDCPA they must actually move forward or it's a violation.

I just checked and I think it is 6 years in my state - (michigan) now I'm in trouble!! 
 
should I sent a pay for delete letter or will it be too late for that?
