PFD example letters?

I'm trying to put together a couple PFD letters. I thought if I searched on here I'd find a few examples. Unfortunately, I keep finding the same letter over and over. Can anyone tell me if I'm searching wrong, or if there is a special place to look.

I hope if dealing with a CA that you sent a DV  CMRRR before thinking about a PFD.

Well, I have 2 medical collections from when I had my son.  I want to pay and have them removed.  Thanks though for double checking me.  As a newbie, I need to make sure I'm going about everything right. I do appreciate all your help Happy!

your more then welcome!!!  you can also contact the provider and IF they still own the account, you might be able to pay them........
