What constitutes an account being listed as "twice"?

I have a CC account listed in my Accounts by the CC Company as a CO, It is also listed under Collections, but put their by a CA and with a different tracking/account number.  Is this considered as being listed "twice" on my report?

I had that on my CR also. I disputed as not mine & they were dropped. I now am waiting for answer to DV letters.  I made sure that SOL (state) was past before I sent DV to CA

I had that happen once, I disputed it and the OC came off.
