Looks like a typical letter. Again see prev. comment on the 30 days and you may need to alter because the balance is $0.
 
I also have to say this....I like to work any and all correspondence based on what the law says, BUT I have no problem at all revising a DV to include items not covered by the law, so as long as I don't expect them to follow the letter to the letter (no pun intended). I think it is OK to add the 30 day statement and other things like requesting proof that they collect is a particular state, and so on. CAs stretch reality and I have no problem reciprocating.
