what is the first step to take and how to go about it?

                                   I need help getting this removed from my credit report, can anyone help me out and tell me what is the first step to take and how to go about it?

Started my credit repair journey on 11/29/13 with 27 collection accounts with 24 being medical related, 1 cellphone account, 1 bank account and 1 cable TV account.
Starting scores are EQ 506, TU 516, EX 477.
First PFD letter sent out on 12/2/13/ Response on 12/5/13 agreeing to the terms of letter.
Second PFD letter sent out on 12/2/13/ Still waiting on a response.

it's a family member's card who added me on so i could used the card but i never did.

My understanding after reading tons of posts here is that authorized users have no financial responsibility. Can this not be disputed as "not mine"?

who do i send the letter to? what should be included in the letter?
 
there is alot of wrong infomation on my credit report;
my address is wrong
my date of birth is wrong
my middle initial is wrong
my job is wrong
 
what should i do?  can all this wrong info help in getting them to remove it from my file?

Use search option for: dispute letter.
 
Lettes should be sent to CRA and (or) Creditors.
 
TU can be easy disputed by phone and online, at least worked for me.
 
EX and EQ are better via mail, altough ine of this two can be disputed on-line too.(my bad can't remember wich one).
 
Most of that personal info do not eeffect your score.
