Property transfer - land deed

My grandmother wants to transfer a small piece of property (no existing structures) into my dad's name. No cash exchanged, she just wants to give it to him. Someone at the court house told her she just needed to do a land deed to transfer it into his name. Is there a blank form somewhere we can use to do this or do we have to get a lawyer to do it? They don't really have any extra money are we are trying to get it done as cheap as possible.

I used legalzoom or something like that for a simple transfer.  Costs $17 or something like that.  Do a probate search for the property to make sure it isnt encumbered etc. (this is what a title company would do)  Finally, if you wanted you could contact a title company and ask them what they would charge to do the paperwork and issue a title, around here it might be as little as $125.
