Question about conditional approval in Texas

I applied for a mortgage earlier this month and was conditionally approved this past Monday. The inspection, appraisal, survey, and title work have all been completed.

I have two questions -

1. My lender said it was fine not to extend the third party financing timeline on the contract, so what will happen now that my 21 days are over if this loan does not approve?

 

and 

2. One of the conditions were to send a letter explaining who the person on my joint checking account was. It is my girlfriend (not married). Will they require that we include her debts? (I am in Texas and this is an FHA loan through Plaza Home Mortgage)

 

any advice or insight is appreciated.
