Looking to get $12k loan with poor credit but great co-signer

Hi all, 

I need a prodedure that goes for $9k-$12k but I have poor credit in the high 500s. I used Ovation which dropped some things off and should put me in the low 600s but I don't know how long it will take to bump the score up.

My co-signer and I have a long standing relationship with Chase Bank but it seems the Chase Health Advance loan is no more. My coisgner is in the 800+ range but I know it may be an uphill battle to be approved for so much with my score.

Any suggestions on which route to take? (AdvanceCareCard.com had NO options when I clicked the "Fair" credit option.

That's exactly what I'm thinking. Trying to do research now on CareCredit. Would like the max time to pay back (60 months).
