I have an outstanding medical bill which at the time I can not pay. It is on my credit report being reported by a debt collector with last update of 4/13; now i get a letter from a completely different debt collector for the same bill stating that they will report me if i do not pay in 30 days? can they do that if the debt is with someone else? or do you think they bought it and will update it in my report. If they update to the most recent date, will it decrease my score??

I would send a DV to the new debt collector. Only one can collect.
This is a catch 22 stiuation. You said you cannot pay. If you send a DV to the CA it puts a cease collections bar prohibiting them from adding it to your CR, until they validate.
If they validate you are open to a law suit if you cannot pay unless it is outside your suing SOL.
