4 years later went to collections?

In 2011 I did not have medical benefits and had a kidney stone. I filled out 'im poor' papers and they paid everything. Apparently they didnt pay the ER fee of $400. almost 4 years later they FINALLY send it to collections and it appears on my equifax report? Is this normal procedure? Can I dispute this?

Unfortunately you have no grounds for a dispute.  If there was a bill that went unpaid, they are within their rights to report.

What you can do is contact the original ER, and see if you can get them to pull it back from collections and accept payment.  That, or the HIPAA letter is your best course of action.
