APPLIED BANK

Hey guys I have a quick question about applied bank. I have a CO from them I ended up paying it in full before I found this message board. I know I should have done a PFD. Has anyone had any luck with a GW with them? Or anything else I can do? If so is there a contact person someone might know of or even an email? I searchded older posts and couldn't find much information on them other than their po box in florida.
