Cap One Collections Mailing Address?

I've had an odd item from Capital One pop up on my report recently as a bad debt item, but there's zero contact information on it, not even an account number. Just a date (from 2006 no less) and an amount.

Would anyone be able to help me with a mailing address for their collections department so I could find out more about this item?

Current Score: TU - 662 (2/14/13) ...... EQ: 677 (4/15/13) ...... EXP: 649 (2/26/13, lender pull)
