What personal info do I include in a DV letter to a CA? Do I sign it?

Never ever sign a DV letter. There have been some cases of CAs scanning the signature for their own use, if you know what I mean.

When I would send my DVs, I would use any number of form letters here and include the CA name and address, and I would go a step further and add the CA's internal account number. Some like the CA to work for this, but I never wanted the CA to respond to say there's a lack of info provided that they couldn't investigate. I will never include the OC amt due, contact info or even the name, or any other identifying info like dates, etc.
