I have a DTE utility on my credit report for close to 4 yrs that was from my budgeted acct. It shows NO lates/ paid as agreed, but I did close the account when my fiancé and I merged our homes.

Is this a good, bad or indifferent effect on my FICO score?

No negatives... leave it be. Let it sit there till it falls off.

July 2013 score: EQ FICO 819, TU08 778, EX "806 lender pull 07/26/2013
Goal Score: All Scores 760+, Newest goal 800+

I was concerned because it said account closed at the request of the account holder.

Nope.. no negative there. Just means you were done with them and closed them out.
