GW Letter--any suggestions?

I have 3 medicals that I paid to the OC and then found about about the ability of the OC to recall the debt.   Well of course they chose not to.  Current dilemma: how do I write an GW letter for these?  I have looked at some on here and they doesn't sound like they fit right.

Use the letters you've found online as a guide...then customize it to fit your situation.  I've had luck when I've kept it brief and honest.

Remember to be polite since you're asking them for a favor.
