Goodwill Letter question

Hello all. Finishing up writing my first goodwill letter. It obviously contains my name, address, partial account #,... but am I supposed to leave an email address, social sec# fax # or telephone # ?

I wouldn't put my Social Security number but I see no harm in an email or telephone number.

If I fax do I need a cover page or should I just fax the letter by itself?

Anything you fax I would have a cover letter just so it gets to where it needs to be.
