How do I remove a person name, from my credit ?

I had given my name for credit support in 2009, however, that was only to hold for that year.  Instead, it was marked as joint, and the person kept moving and my address seemed changing according to his.  I did not move.  I do want to have this persons name and addresses removed from my credit. how to go about it.

What is marked as joint?  A credit card,bank account or a loan?

Did you talk with the OC (orginal creditor) about this?

Do you have a current copy of your CR's (credit reports)?
