I have a collection which says date open 10/1/2011 for a medical bill that I believe my insurance should have covered. Does anyone know how far back insurance companys will go to pay for a medical bill?"
That probably depends on your insurance and the circumstances that lead to it being sent to collections. Meaning, were they not billed or not correctly?
I would still try.
I dont believe they were billed. Its a low amount of money I was in the process of sending the CA a PFD, I talked to a rep on the phone and they told me they dont do that but would not let me talk to a supervisor so maybe a formal letter will be better.
A PFD isn't really a formal letter. It is like a GW letter so keep it nice with no demands.

Not necessarily.

Yes this is true. Or any reference that could tell others what type of procedure you had or somehow identify an illness.
But having a collection for a medical bill is not in and of itself, a violation of HIPAA.
