Information to submit with GW letter?

Was curious as to what information I should include in my GW letter?

Just Name, Address, Phone Number?  Or do I put my social security and account number in their also?  Side note my accounts were closed a few years back.

I do name, address, phone number, and account number.  I would leave out the ss number.

+1, they can find your file with Name, Address, Phone and Account Number.

No need to give them more info, like your SS#.
