You will find many, many flavors of DV letters, both on this site and on others.
The basic issue you face in content of a DV letter is your interpretation of what the statute requires in order to provide adequate debt verification.
The four-corners of the statutory language of FDCPA 809(b) only stipulates that they debt collector obtain verification, and pass that finding on to the consumer, along with the amount of the debt, and if requested, the name of the original creditor.
You will find many DV letters that require a long list of items, such as documentation from the OC, under the umbrella of what is commonly called "full media verification."
Most courts do not require these items, but some have interpreted the language of section 809(b) to require such items.
If you wish to require more, then be prepared to take them before a judge who might make such an interpretation of the statute.
My personal recommmendation is a simple
"This letter is a request for verification of the debt under FDCPA 809(b)," with the addtion, if also sought, of the request for the name of the original creditor.
