Medical Collection for services never rendered

So my DW has a medical collection for services that were never performed. 

 

The facility will claim that they were performed.

 

How does one go about tackling this one? I guess I am kind of at a loss as to where to start. 

Re-entered the garden on 9/16/2013.

Asking for procedure notes, lab values, and other signs they've done something might be a good way to start.
