My med bills on my spouse's report

This is happening to me.  I now understand they can go after me even though its my wife's debt, but do they have to notify me first? ... I got nothing from them until I pulled a credit report and it was on there.

If you live in a community property state, both spouses are equally liable and can be on both CR, no matter who incurred the debt.

Not saying you do live in a community property state.

understood, but does the collection agency have to attempt to collect debt from me first?  I would think since they are allowed to put it on my credit, they would also be obligated to follow the same rules (notify my in writing, etc)....?

They can put it on your CR without first notifying you.  They are supposed to send a notice within 5 days if they do.
