Question about sending deletion letters/orders to set aside judgment

I recently obtained an Order to Set Aside Judgment from a judgment I paid and a Deletion letter for a paid in full collection account. I know to send copies of these to the three CRAs, but what else do I include with the copies? Should I write down my SSN, date of birth, address, or any other info? I'm assuming I don't just send them blindly with nothing to refer to, but I don't know the process.

How did you go about getting the Order to Set Aside Judgement? I would like to know because I have a PIF judgement from 2008 that I would loooove to get off my report....
