Sending a DV to a CA for my dentist bill

I am not sure what to do about this situation.... Here is the story

There is a CA on my CR's for $150.00 from my dentist.  This should have been paid for by my insurance and they will not pay this.  I have sent several PFD's to both the dentist office and the CA.  They both say no will not delete for payment!

My dentist office is no longer in existence.  Should I try to send a DV to the CA? 

This has been listed on my CR from 1/2007 and the SOL for my state is 2 years.

Please recommend what I should do.

Dentist come under HIPAA.  Where did you get the 2 year SOL?

OK for HIPAA do you have to pay the bill to get it removed?  Also I read some where that for my state is 2 years for SOL.

Depending on which state you live in, SOL on non-hospital med debt is 3 or 4 years. I would google to get help with this.
