What Action Get a Tax Lien on Our Credit Report

Since being so focused on recovery, I have seen a lot of messages about tax liens.  I have no experience with these.  I was just wondering what actions/activity gets a tax lien on our credit report?  Is it anytime the feds or state believe we owe them money, or is it only if we avoid/ignore them?

Thanks for any insight!

Lien is last resort after call other options been exhausted. If you talk with them to work out your differences there is no lien filed.
