home advantage medical

On both DH and I cr we have something from home advantage medical. I have never had anything from them. Hubby did for sleep apnea machine. Not sure why its on both our cr's. This place is now permanently closed! advice on getting it removed from cr's? dispute?

Before doing anything else google the HIPAA Letter Process follow it exactly as laid out.  If you have any problems finding the website or have questions PM me.
