need HIPPA help - billed twice for same item by same company

I have a few nasty medical bills from 2008 and notice that one of the companies is billing me the exact same amount $5,641 with two different account numbers. I am sure this is an error (I had surgery but dispute double charge) and I am wondering if someone could kindly message me the pre HIPPA letter to see if that can help with some of my medical bills.
