I actually prepared a PTD letter for them today myself!!!  Mine is from December 2011, for $496.  I used the address that showed on my credit report, which is PO Box 965036 Orlando, FL  32896 5036.  Hope that helps!!  Let me know if you get a response!

I was going to say that when trying a PFD, if possible, you want something in writing just in case.
