I'm not sure about your state's process, but where I live you can submit all of your claims at once. So, while I had one for $9, that wouldn't have been worth my time, but with all of my claims together, I threw it in.
For that state, it's one form per claim, submitted with all proofs (Identity and relationship with company).
I found two for me - one under current name and one under former name. So definitley two papertrails. And one for DH. So three papertrails. And I am hoping we have proof of addresses going back more than 10 years!
Maybe they can all be mailed in the same envelope. Still, any additional funds will be nice. I am glad you had posted this. It's been awhile since I have looked.
I lucked out because I only had two addresses: my current address and our home that we moved from 11 years ago. For that, I was able to order an old W-2 from my work for 10 cents. It was worth it because the funds were over $100 for that claim.
I did find about $100 from my deceased uncle. Since that estate was split 50% to my aunt and then 50% with my six siblings, one of whom has since passed, I would be entitled to 10 plus 5% of my brother's share, so $10.50. Not worth it for any of us.
For my husband, I found over $1000 for his deceased parents. That one will be worth it ($250+ for each sibling). I notified my BIL, who was executor of the estate. We will see if anything comes of it.

I submitted my claim in early November. It has been received by the state but the payment can take up to 6 months. So I guess by April I'll get the funds.
Found over $660, just printed/made copies of all the required docs and sending it in the mail tomorrow.
An unspecified sum (I'm guessing maybe $10 or $20) in unredeemed Starbucks gift cards.
Capital One Bank owes me 34 cents. This must be 20 years old. Can I add interest? I still live in the same house.
