Does your credit score increase sometimes while you are disputing an item (particularly equifax but was wondering for all three). It seems that Equifax will not consider those items while it's under review in regards to score and if it deletes, great but if they state valid then it goes back down. Is it just me? I am dealing with some old medicals.

Portions of an OC account under dispute will not be scored by FICO. Among these are the balance, the CL, and the payment history. Other items like AAoA, scoring bucket, credit mix, account age, etc. including CAs and PRs are not removed or impacted by a dispute.
So yes scores can increase while a dispute runs it's course.

Yes this includes any of the 3 that you file a dispute with
However be aware that if the dispute comes back as verified your scores would then return to the previous levels
