Young and in Serious Need of Help[ Edited ]

I really need some advice on bankruptcy and I didn't know where else to go, so I hope that some of you good people can come to my aid.
I'm 21 years old, I've been on my own since I was 16, and the past two years have been, for a lack of better words, rough. I live in Indiana, and I have a student loan that I am behind on (but if I could get my other dept taken care of, I believe that I could catch up on it), I had to have a very serious surgery and I now have a medical bill of $17,000, and I am trying to catch up on a payday loan. I work full time, make minimum wage, and pay my rent, electric bill, and phone bill every month, and use what is left for food. I do not have internet, cable, or anything like that, and I own next to nothing. I have a futon, my mattress, a couple tables, the TV I grew up with, books, and the same clothes I've had since I was 18. I looked into bankruptcy (I am still researching as well), and I think that I need to file Chapter 7, but I can't afford it. I know that there are people out there who are worse off than I am, I really do, but I'm scared, I'm broke, I'm on my own, and I don't know what to do. If anyone has any advice....thank you.
