Error when uploading an image

I have been trying to upload an image and it fails.....I keep getting Error Code: IO Error.

I've tried gif, png , jpg file types, same issue. I'm clicking on the insert/edit image button, and I select the file, and it always gives the same error..... What am I doing wrong ??
