Filled it out and got my wife her card back as well.. we travel frequently leasure and business.. no fancy message or anything I dont care just wanted it back be 10 years in June since we filed BK .. so thankful with this forum.. here's the message .. so i guess i dont get a fancy box this time? hahaha Thank You!
